I, being of 18 years of age or older do hereby give permission for my child, to participate in the Tersak's Family Martial Arts After School Program on the school property of their academic school. I hereby release, forever discharge and agree to hold harmless Tersak's Family Martial Arts and the employees and owners thereof from any and all liability, claims or demands for personal injury, sickness or death, as well as property damages and expenses, of any nature whatsoever which may be incurred by my child while participating in the above described activity. I understand that these activities involve a risk of injury. Furthermore, I hereby assume all risk of personal injury, sickness, death, damage and expense as a result of my child’s participation in recreation activities involved therein. I understand that, by allowing my child to participate in karate and its activities with and around other people, including other children, there is a risk that my child may be injured and/or may contract a contagious disease, including but not limited to Coronavirus (COVID-19). I hereby accept that risk and waive all claims, on behalf of myself and my child, against Tersak’s Family Martial Arts Academy, including its owners, agents, employees, volunteers, sponsors, officers, instructors, business partners, and members (collectively, “Tersak’s”), for all claims of injury, illness, or death, that any child may sustain, contract, or incur, even if those claims arise out of the negligence of Tersak’s.
